Passport Centers

Apply for a passport at the library so you can travel abroad. Learn about walk-in services, what to bring, and fees before you embark on your adventure. Passport applications are accepted at East Columbia and Glenwood Branches, on behalf of the U.S. Department of State.

Passport Process & Requirements

Note: All applicants must appear in person, regardless of age. Walk-in only. Appointments not necessary.

1. Application Form (DS-11)

  • A completed DS-11 application form
  • Use black ink only
  • Single-sided printing
  • Do not sign the form before your appointment
  • Leave the area below “STOP” blank
  • A Social Security number is required

 

2. Proof of U.S. Citizenship

  • U.S. birth certificate (must include parent(s) names, official seal, and signature)
  • Naturalization certificate
  • Previously issued U.S. passport
  • Consular Report of Birth Abroad

Note: Original documents only, no photocopies.


 

3. Proof of Identity

For Adults & Guardians:
  • Valid driver’s license
  • State ID card
  • Military ID
  • Passport (for adults applying for a minor)

Not Accepted: Learner’s permits or temporary/restricted licenses.

For Children Ages 0–15:
  • Must appear with all parents listed on the birth certificate.
  • Proof of relationship required (eg. Birth certificate, adoption papers, custody documents or guardianship papers).
  • If one parent cannot attended, a notarized DS-3053 form and a copy (front and back) of the ID presented to the notary is required.
  • Parents must show valid ID; if the name differs from the birth certificate, bring legal proof of name change (e.g., marriage certificate or court order).
For Ages 16–17:
  • Applicant must appear with at least one parent or guardian.
  • Bring one of the following: U.S. birth certificate, naturalization certificate, or previously issued U.S. passport.

Note: All family members must be present at check-in to sign in together.


 

4. Passport Photo

  • One 2″ x 2″ photo with white background required
  • Available on-site: $15 per photo
  • Payment to Howard County Library System by Visa, Mastercard, exact cash, or check
  • For infants: Bring a white blanket and a carrier/car seat

Note: Library photos cannot be used for the U.S. Department of State’s online renewal portal (travel.state.gov), which requires different dimensions and does not accept scanned or printed photos.


5. One Check or Money Order Per Applicant

Payable to the U.S. Department of State.

 

6. $35 Acceptance Fee (Per Applicant)

Payable to Howard County Library System. Visa, Mastercard, exact cash, or check accepted.

 


 

7. Special Circumstances

  • If you have held a passport under a different name, please bring evidence to support the name change (marriage certificate or court order).
  • If a child derives citizenship through parents: Bring
    • Parent’s naturalization certificate
    • Child’s permanent resident card
    • Child’s birth certificate
    • Parents’ marriage certificate
    • Certified English translation if documents are not in English

For urgent travel, call  1-877-487-2778 to make an appointment with the U. S. Department of State. 

The Library isn’t able to process passport renewals with Form DS-82. If you’re eligible to use this form, you’ll need to mail it in yourself.
